A touchless platform that renders the patient-specific 3D biomodel on an auxiliary screen during robotic surgery. It supports selective vascular control, clamping planning and a shorter warm-ischemia time, with no contact with the sterile field.

Why it matters

What changes when the biomodel enters the OR.

In partial nephrectomy for complex tumors, seeing the kidney, its vessels and the mass in a patient-specific three-dimensional model helps anticipate clamping and preserve as much healthy kidney tissue as possible. The details of each case are discussed at the consultation.

MIRAI 3D is an external alliance: the volumetric navigation is provided by a third party, and at UMIBA we integrate it as support for surgical planning when the case warrants it.

The process begins with the patient's contrast-enhanced CT. The MIRAI 3D team processes the image, segments the kidney, arteries, veins and tumor mass, and builds a volumetric biomodel. Before surgery, the biomodel is validated with the surgeon in a dedicated session.

In the OR, the auxiliary screen shows the biomodel. The surgeon manipulates it with touchless gestures, which preserves the sterile field. Rotation, zoom and overlay with the robotic endoscopic image are immediate. Selective-clamping planning is confirmed or adjusted before proceeding.

3D planning before operating on a complex kidney is not a luxury. It is the difference between estimating the clamping and knowing it in advance.

Clinical application

When it adds the most value.

The primary indication is partial nephrectomy in complex tumors: central location, proximity to hilar structures, an anatomically or functionally solitary kidney, synchronous bilateral cases. In simple surgery, the biomodel's contribution is marginal and the cost is not justified.
